Determining factors of communication effectiveness in nursing teams

The quality and effectiveness of nursing care depends on, among other factors, communication processes configuration between health care team members. The objectives of this study were: I) to determine how nurses evaluate the diffe-rent dimensions of the communicative process; II) to identify the main obstacles to communication; III) to gauge how nurses evaluate the adequacy of different com-munication channels in health services. This was an investigation of quantitative descriptive and correlational. A structured questionnaire was developed to obtain relevant data on the matter. The research was carried out in four health services (Neonatology, Medicine II, Emergency and Basic Emergency) and a total of 75 nur-ses were surveyed. A comparative analysis between the four services revealed no statistically significant differences in how nurses evaluate the communicative pro-cess. This fact indicates that the way in which communication is configured and evaluated is transversal to nursing practice and independent from the context in which it occurs. The research reveals the generally positive way how nurses evalua-te the communication process. However, some weaknesses of communication in the clinical context are identified, mainly related to the mode in which information of organizational nature is disseminated within each department

A gestão da segurança dos cuidados, através da melhoria dos registos de enfermagem no serviço de urgência / Care safety management, through the improvement of nursing records in the emergency department

O presente relatório contempla o percurso realizado para o desenvolvimento de competências específicas em Enfermagem Avançada na área de Gestão de Unidade de Saúde, fazendo uma análise reflexiva das competências adquiriras e desenvolvidas do Enfermeiro Gestor. A área de intervenção definida foi a Gestão da Segurança dos cuidados através da melhoria dos registos de Enfermagem no Serviço de Urgência. Objetivo: Garantir a segurança dos cuidados através da melhoria dos registos de Enfermagem no Serviço de Urgência, Metodologia: Recorreu à metodologia do planeamento em saúde respeitando todas as fases que lhe são inerentes. Para o diagnóstico da Situação foi aplicado uma Lista de Verificação dos registos de Enfermagem com Análise documental como técnica para o tratamento dos dados e realizou uma Entrevista aos enfermeiros do Serviço de urgência. Resultados: Constatou a ausência de registos de Enfermagem uniformizadas para assegurar uma comunicação segura e que o Sistema de informação utilizado, tendo em conta o fluxo de trabalho e tempo necessário disponibilizado não é facilitador de registos de Enfermagem seguros. Conclusão: A contratualização da elaboração de registos de enfermagem, através da avaliação de desempenho, de acordo com uma Instrução de trabalho - modelo orientador de boas práticas, facilita a comunicação eficaz entre a equipa e garante a gestão da segurança dos cuidados.

This report considers the course carried out for the development of specific competences in Advanced Nursing in the area of Health Unit Management, making a reflexive analysis of acquired and developed competencies of the Nursing Manager. The defined intervention area was the management of care´s safety, through the improvement of the Nursing Records in the Emergency department. Aim: Ensure the care´s Safety by improving nursing records in the Emergency Department Methods: It was used the health planning methodology, respecting all the phases that are inherent to it. For the diagnosis of the Situation, a Checklist of Nursing records with Documentary Analysis was applied as a technique for the treatment of the data as well as an Interview to the nurses of the Emergency Department. Results: It was verified that the absence of standardized Nursing records to ensure a secure communication also that the information system used, taking into account the workflow and the available time necessary, is not provinding a safe Nursing records. Conclusion: The Contracting of the elaboration of nursing records, through performance evaluation, according to a Work Instruction - guiding model of good practices, facilitates effective communication between the nursing team and guarantees management of care´s safety.
